Thyroid & Hashimoto's Care in Westfield, NJ
Endocrinologist-led thyroid care, in person or by telehealth in NJ, NY & FL
EndoHealth MD offers thyroid care led by Dr. Alessia Roehnelt, a double board-certified endocrinologist who is ECNU-certified in neck ultrasound. Care covers Hashimoto's thyroiditis, hypothyroidism, hyperthyroidism, and thyroid nodules.
A normal TSH does not always mean you feel normal. We look at your symptoms, your full thyroid labs, and the rest of your hormones and metabolism before choosing a treatment.

Is This Right for You?
Common reasons patients come to us
You may be a good fit if you are tired, cold, losing hair, gaining weight, or feeling foggy, and you have been told your labs are fine. We also see patients with Hashimoto's or Graves' disease, thyroid nodules found on a scan, a family history of thyroid disease, or thyroid changes during pregnancy planning or after having a baby.

Many of our patients have been on the same dose for years without anyone asking how they feel. We take the time to look at the whole picture.
How We Evaluate Your Thyroid
Labs, symptoms, and your story
Care begins with a 60-minute consultation with Dr. Roehnelt. She reviews your symptoms, medications, supplements, family history, and past labs and imaging. Testing is chosen for you and may include TSH, free T4, free T3, and thyroid antibodies, plus iron, vitamin B12, and vitamin D when symptoms overlap.

If you have a thyroid nodule or an enlarged thyroid, Dr. Roehnelt reviews your imaging and explains your next steps. Tip: stop biotin supplements 2 to 3 days before labs, since biotin can throw off thyroid test results.

Hashimoto's and Thyroid Treatment
Matched to your health, not a one-size plan
Hashimoto's thyroiditis is the most common cause of hypothyroidism in the United States. It happens when the immune system slowly damages the thyroid. Not everyone with Hashimoto's needs medication right away. When the thyroid can no longer keep up, the standard treatment is levothyroxine (T4), adjusted until your labs and symptoms improve.

Some people still have symptoms on levothyroxine alone. In those cases, Dr. Roehnelt may discuss adding liothyronine (T3) or other options, based on current guidelines and your health history.

She also treats hyperthyroidism and Graves' disease, and follows thyroid nodules over time.

Beyond the Thyroid
Weight, energy, and the rest of your hormones
Thyroid problems often travel with other issues. Weight gain, insulin resistance, perimenopause, low iron, and poor sleep can all cause symptoms that look like a thyroid problem, and treating the thyroid alone may not be enough.

As an endocrinologist, Dr. Roehnelt looks at all of these together. If you also need help with weight, hormones, or blood sugar, it becomes part of one plan.

Common Questions
Short answers to what patients ask most
Can I have Hashimoto's with a normal TSH? Yes. Antibodies can be positive for years before TSH changes. Dr. Roehnelt will decide how often to recheck and whether treatment makes sense for you.

What is the difference between Hashimoto's and hypothyroidism? Hashimoto's is an autoimmune condition that attacks the thyroid. Hypothyroidism means the thyroid makes too little hormone. Hashimoto's is the most common cause of hypothyroidism, but you can have Hashimoto's while your thyroid levels are still normal.

How should I take my thyroid pill? Levothyroxine works best on an empty stomach with water, 30 to 60 minutes before breakfast or coffee. Take calcium and iron at least 4 hours apart from it.

Will a gluten-free diet cure Hashimoto's? No diet has been shown to cure Hashimoto's. Celiac disease is more common in people with Hashimoto's, so Dr. Roehnelt may test for it and can help you decide whether a diet change is worth trying.

Can I do this by telehealth? Yes. Dr. Roehnelt sees patients virtually in New Jersey, New York, and Florida.
